Process for Instructors to align with College of Medical Training

1. Complete Training Site Registration (not applicable for individual Instructors)

2. Each instructor to complete a Registration Agreement

3. Each instructor to complete a Data Form

4. Each instructor is to complete a Transfer Request

5. Each instructor is to submit copies of all their certificates for each discipline they teach

6. Each instructor to submit copies of the AHA update

7. TS must submit a copy of  the policy and procedures for cleaning and maintaining manikins.

8. Copy of TS general liability insurance coverage or waiver to be submitted to TC.

9. TS must submit a copy of administrative policy and procedure that address quality assurance, continuous quality improvement, internal dispute resolution, and management of TS communication and updates.

10. Pay Instructor Registration Fee (20.00 per Instructor)
